          Abstract

          Um dos exercícios teóricos da Saúde Pública diz respeito à necessária e contínua tarefa de se discutirem os conceitos nos quais se ancoram suas práticas. Uma análise crítica sobre os usos do conceito de vulnerabilidade em saúde, com base em uma revisão sistemática, serve de suporte para uma discussão sobre seu conteúdo, alcances e limites, no sentido de fortalecer a potencialidade teórica e prática do conceito e o diálogo entre as distintas áreas de conhecimento implicadas. O conceito de vulnerabilidade apresenta alta capacidade heurística e aplicação diferenciada. Neste artigo, ele é caracterizado com base em complexos processos de fragilização biossocial que exprimem, de maneira inextrincável, valores biológicos, existenciais e sociais. Esta perspectiva considera vulnerabilidade como dimensão ontológica constitutiva e constituinte da vida humana, que reclama distintos e complexos sistemas de segurança.

          Translated abstract

          One of the theoretical exercises relating to public health comprises the necessary and continuing task of discussing the concepts that underlie its practices. A critical analysis on the uses of the concept of vulnerability in relation to health, based on a systematic review, provides the underpinning for a discussion on its content, scope and boundaries, with the aim of strengthening the theoretical and practical potential of the concept and the implied dialogue between the different fields of knowledge. This concept has high heuristic capacity and can be applied in different fields. In this article, it is characterized based on the complex processes of biosocial fragility that inextricably express biological, existential and social values. This perspective considers vulnerability to be an ontological dimension constitutive of human life that necessitates a diversity of complex security systems.

          Uno de los ejercicios teóricos de la Salud Pública se refiere a la necesaria y continua tarea de discutir los conceptos en los que se basan sus prácticas. Un análisis crítico sobre los usos del concepto de vulnerabilidad en salud, con base en una revisión sistemática, sirve de soporte para una discusión sobre su contenido, alcances y límites, en el sentido de fortalecer la potencialidad teórica y práctica del concepto y el diálogo entre las distintas áreas de conocimiento envueltas. El concepto de vulnerabilidad presenta alta capacidad heurística y aplicación diferenciada. En este artículo, se caracteriza con base en complejos procesos de fragilidad bio-social que muestran, de forma inextricable, valores biológicos, existenciales y sociales. Esta perspectiva considera la vulnerabilidad como dimensión ontológica constitutiva y constituyente de la vida humana que reclama distintos y complejos sistemas de seguridad.

            Frailty: an emerging concept for general practice.

            Ageing of the population in western societies and the rising costs of health and social care are refocusing health policy on health promotion and disability prevention among older people. However, efforts to identify at-risk groups of older people and to alter the trajectory of avoidable problems associated with ageing by early intervention or multidisciplinary case management have been largely unsuccessful. This paper argues that this failure arises from the dominance in primary care of a managerial perspective on health care for older people, and proposes instead the adoption of a clinical paradigm based on the concept of frailty. Frailty, in its simplest definition, is vulnerability to adverse outcomes. It is a dynamic concept that is different from disability and easy to overlook, but also easy to identify using heuristics (rules of thumb) and to measure using simple scales. Conceptually, frailty fits well with the biopsychosocial model of general practice, offers practitioners useful tools for patient care, and provides commissioners of health care with a clinical focus for targeting resources at an ageing population.
