      Central odontogenic fibroma (simple type) in a four-year-old boy: atypical cone-beam computed tomographic appearance with periosteal reaction

          Abstract

          Central odontogenic fibroma (COF) is a rare benign tumor that accounts for 0.1% of all odontogenic tumors. A case of COF (simple type) of the mandible in a four-year-old boy is described in this report. The patient showed asymptomatic swelling in the right inferior border of the lower jaw for one week. A panoramic radiograph showed a poorly-defined destructive unilocular radiolucent area. Cone-beam computed tomography showed expansion and perforation of the adjacent cortical bone plates. A periosteal reaction with the Codman triangle pattern was clearly visible in the buccal cortex. Since the tumor had destroyed a considerable amount of bone, surgical resection was performed. No recurrence was noted.

            Radiologic features of central odontogenic fibroma.

            The central odontogenic fibroma is a rare benign neoplasm that is considered to be derived from mesenchymal dental tissue. This study reviews and analyzes the radiologic features of 51 cases (5 new cases and 46 from the literature). Central odontogenic fibroma appears in both the mandible and maxilla (55% and 45%, respectively). In the maxilla it has a tendency to involve the anterior area, whereas in the mandible the molar and premolar areas are the most prevalent sites. The majority of central odontogenic fibromas are unilocular radiolucent lesions with well-defined borders, but they may also appear as multilocular lesions and in rare instances may exhibit a mixed radiolucent/radiopaque appearance with poorly defined or diffused borders. The great variability in radiologic appearance of the central odontogenic fibroma means that it should be considered in the differential diagnosis of all radiolucencies found in the jaws.

              Central odontogenic fibroma of mandible: a case report and review of the literature.

              Central odontogenic fibroma is an extremely rare benign tumor, accounting for less than 0.1% of all odontogenic tumors. The most recent literature review revealed that only 69 cases have so far been reported in the English literature, and only 1 of these cases showed radiologic appearance of a dentigerous cyst and this involved impacted maxillary incisors. A case of central odontogenic fibroma occurring in the mandible of a 30-year old female with radiological appearance of a dentigerous cyst involving an impacted lower first molar, presented here, appears to be the first reported case of central odontogenic fibroma of mandible with radiological appearance of dentigerous cyst in the English-language literature.
