      Caries dental en primeros molares permanentes y factores socioeconómicos en escolares de Campeche, México

          Abstract

          El objetivo de este estudio fue determinar el estado de caries en los primeros molares permanentes para analizar la severidad del daño causado, la magnitud y distribución del problema, así como explorar su relación con factores socioeconómicos. En una muestra representativa de 3 615 escolares de 6 a 13 años de edad, 3 examinadores estandarizados (Kappa = .95) aplicaron el criterio de magnitud de la lesión cariosa (Gutiérrez, 1987). La prevalencia de caries fue del 80,3 %, el CPOD para el grupo de 6 años fue de 0,11 (.444) y para los de 12 de 1,25 (1.78). La frecuencia de escolares con lesiones más severas de caries fue del 17,37 %. El porcentaje de hombres con los 4 primeros molares afectados es de 1,47 % en el grupo de 6 a 9 años y de 5,67 % en el grupo de 10 a 13 años; este porcentaje se incrementa de 1,11 a 6,5 % en las mujeres en los mismos grupos de edad. Se encontró asociación entre la frecuencia de niños con lesiones severas y la actitud de la madre hacia la salud oral, número de hijos en la familia y escolaridad de la madre. Los resultados indican la persistencia de una proporción de la población con altos índices de caries y la necesidad de educación acerca de la importancia de este diente y de la salud oral.

          Translated abstract

          This study was aimed at determining the state of caries in the first permanent molars to analyze the severity of the damage caused, the magnitude and distribution of the problem, and to explore their relationship with socioeconomic factors. In a representative sample of 3 615 schoolchildren aged 6-13, 3 standardized examiners (Kappa = 95) applied the criterion of magnitude of the carious lesion (Gutierrez, 1987). The prevalence of caries was 80.3 %, the CPOD for the group aged 6 was 0.11 (.444), whereas for those aged 12 it was 1.25 (1.78). The frequency of school children with more severe lesions was 17.37 % The percentage of men with the first 45 molars affected is 1.47 % in the group aged 6-9, and 5.67 % for those aged 10 - 13. This percentage increased from 1.ll to 6.5 % among women from the same age groups. It was found association between the frequency of children with severe injuries and the mother's attitude towards oral health, number of children in the family and the mother's educational level. The results showed the persistence of high indexes of caries and the need of education about the importance of this tooth and of oral health.

          First permanent molar: first indicator of dental caries activity in initial mixed dentition.

          The objective of the present study was to investigate among children in the initial mixed dentition phase the presence of clinical signs that might eventually function as more sensitive indicators of the development of caries disease, denoted here as caries activity. On this basis, we investigated the relationship between salivary levels of mutans streptococci (MS) and decayed, missing and filled permanent and deciduous tooth surfaces (DMFS and dmfs) using microbiological, clinical and radiographic examinations in 81 schoolchildren aged 7-8 years. Whereas dmfs did not present a positive correlation, DMFS was significantly correlated with salivary MS levels. The first permanent molars of the schoolchildren studied comprised 87.3% of the affected surfaces recorded in the DMFS, suggesting that the development of new lesions was preferentially located on the surfaces of the first permanent molars. These results permit us to conclude that the first permanent molars function as first indicators of dental caries activity in the schoolchildren examined.
