      Epidemiological aspects of leprosy in Juazeiro-BA, from 2002 to 2012*

          Abstract

          BACKGROUND

          Leprosy is an infectious disease caused by Mycobacterium leprae , able to infect large numbers of people. This work is relevant to Juazeiro-BA, a hyper-endemic area for leprosy, since unravel the behavior of the disease in the area, may suggest the decision making for sectors of surveillance, establishing strategies, organizing and evaluating programs and services.

          OBJECTIVES

          To analyze the epidemiology of leprosy in Juazeiro-BA, from 2002 to 2012.

          METHODS

          A descriptive, cross-sectional study was conducted based in data of the Diseases Notification System, assigned by the service of Epidemiology from Juazeiro-BA, between 2002 and 2012.

          RESULTS

          1,916 new cases of leprosy were detected between 2002 and 2012, of which 921 (48.07%) represented male sex, 995 (51.93%) female, and there was a reduction in the incidence rate of leprosy per 100,000 inhabitants. Most carriers were brown individuals, with low levels of education, living in the urban area, being more prevalent in the economically active age group. Through statistical analysis we found that there are more chances of developing sequelae among men, and multibacillary individuals older than 45 years.

          CONCLUSIONS

          The work serves to direct efforts to control this disease, and highlights the importance of active search for new cases to achieve an early diagnosis, reducing the number of sequels and allowing breaking the chain of disease transmission.

          Socioeconomic, environmental, and behavioural risk factors for leprosy in North-east Brazil: results of a case-control study.

          Brazil reports almost 80% of all leprosy cases in the Americas. This study aimed to identify socioeconomic, environmental, and behavioural factors associated with risk of leprosy occurrence in the endemic North-eastern region. A case-control study in four municipalities. cases of leprosy diagnosed in the previous 2 years, with no other known, current, or past case of leprosy in the household or in the neighbourhood. individuals presenting for reasons other than skin problems to the health unit where the case was diagnosed and who lived in the same municipality as the case with whom it was matched. For each case four controls were selected. A semi-structured questionnaire was used to collect demographic, socioeconomic, environmental, and behavioural data. A multivariate hierarchical analysis was performed according to a previously defined framework. 226 cases and 857 controls were examined. Low education level, ever having experienced food shortage, bathing weekly in open water bodies (creek, river and/or lake) 10 years previously, and a low frequency of changing bed linen or hammock (>or=biweekly) currently were all significantly associated with leprosy. Having a BCG vaccination scar was found to be a highly significant protective factor. Except for BCG vaccination, variables that remained significant in the hierarchical analysis are cultural or linked to poverty. They may act on different levels of the transmission of Mycobacterium leprae and/or the progress from infection to disease. These findings give credit to the hypothesis that person-to-person is not the only form of M. leprae transmission, and that indirect transmission might occur, and other reservoirs should exist outside the human body.

              Perfil epidemiológico da hanseníase em um município brasileiro, no período de 2000 a 2006

              INTRODUÇÃO: A hanseníase é considerada um grande problema de saúde pública nos países em desenvolvimento. Estima-se que somente 1/3 dos doentes sejam notificados e que, dentre esses, muitos fazem tratamento irregular ou o abandonam, aumentando o impacto da doença. Assim o objetivo desse artigo foi descrever o perfil epidemiológico da população com diagnóstico de hanseníase, no município de Uberaba, Estado de Minas Gerais, Brasil, no período de 2000 a 2006. MÉTODOS: Trata-se de um estudo retrospectivo, que utilizou os dados secundários de notificação de casos hanseníase do Sistema de Informação de Agravos de Notificação do Ministério da Saúde do Brasil. RESULTADOS: Foram registrados 455 casos da doença, sendo 55,4% do sexo masculino, a faixa etária dos 34 a 49 anos (31,4 %) foi a mais afetada, houve registro de nove (2%) casos de hanseníase em menores de 15 anos. A forma clínica prevalente foi a dimorfa (69,1%) e a classe operacional foi a multibacilar (87%). Tais achados são preocupantes, considerando-se que são de faixa etária economicamente ativa e potencialmente, os principais disseminadores da doença. CONCLUSÕES: O relato de que a maioria dos casos eram multibacilares, indica diagnósticos tardios, assim, torna-se necessário descentralizar o serviço de hanseníase e capacitar mais profissionais para possibilitar diagnóstico e tratamentos mais precoces.
