          Abstract

          Resumo Enquadramento: a Diabetes Mellitus tipo 1 é uma doença crónica que afeta maioritariamente crianças e jovens. No seu tratamento a insulinoterapia é essencial. A insulina pode ser administrada por Múltiplas Injeções Diárias (MID) ou por sistema de Perfusão Subcutânea Contínua de Insulina (PSCI). Este último tem vantagens, mas o processo de adaptação é exigente. Objetivo: conhecer a perceção das mães relativa ao sistema de PSCI no tratamento da DM1 dos adolescentes. Metodologia: estudo qualitativo descritivo e exploratório. A recolha de dados foi realizada por entrevista semiestruturada a 10 mães de adolescentes com DM1 com tratamento por PSCI. A análise foi realizada pelo método de análise de conteúdo de Bardin. Resultados: da análise das entrevistas emergiram quatro categorias: A transição para PSCI, Bem-estar do adolescente e família, Autonomia do adolescente e Limitações do dispositivo e respetivas subcategorias que expõem a perceção das mães de adolescentes com DM1 sobre tratamento com PSCI. Conclusão: o tratamento com PSCI proporciona melhor qualidade de vida ao adolescente e à sua família e favorece a autonomia dos adolescentes. O processo de adaptação coloca-lhes desafios sendo fulcral o suporte dos profissionais de saúde. Identificaram-se limitações no dispositivo que se desejam ultrapassadas atendendo à inovação tecnológica atual.

          Resumen Marco contextual: la diabetes mellitus tipo 1 es una enfermedad crónica que afecta principalmente a niños y jóvenes. En su tratamiento la terapia con insulina es esencial. La insulina se puede administrar mediante inyecciones diarias múltiples (MID) o mediante el sistema de infusión continua de insulina subcutánea (ICIS). Este último tiene ventajas, pero el proceso de adaptación es exigente. Objetivo: conocer la percepción de las madres respecto al sistema ICIS en el tratamiento de la DM1 de adolescentes. Metodología: estudio cualitativo descriptivo y exploratorio. La recolección de datos se realizó mediante entrevista semiestructurada con 10 madres de adolescentes con DM1 que se sometieron a tratamiento con ICIS. Su análisis se realizó mediante el método de análisis de contenido de Bardin. Resultados: del análisis de las entrevistas surgieron cuatro categorías: La transición al ICIS, Bienestar del adolescente y la familia, Autonomía del adolescente y Limitaciones del dispositivo y respectivas subcategorías que exponen la percepción de las madres de adolescentes con DM1 sobre el tratamiento con ICIS. Conclusión: el tratamiento con ICIS proporciona una mejor calidad de vida a los adolescentes y sus familias y favorece la autonomía de los adolescentes. El proceso de adaptación plantea desafíos y el apoyo de los profesionales de la salud es fundamental. Se identifican algunas limitaciones en el dispositivo que se desea superar dada la innovación tecnológica actual.

          Translated abstract

          Abstract Background: type 1 Diabetes Mellitus is a chronic disease that affects mostly children and young people. In its treatment, insulin therapy is essential. Insulin can be administered by Multiple Daily Injections (MDI) or by continuous subcutaneous insulin infusion (CSII) system. The latter has advantages, but the adaptation process is demanding. Objective: to identify mother’s views regarding the PSCI system in the treatment of DM1 of adolescents. Methodology: qualitative descriptive and exploratory study. Data collection was performed by semi-structured interview with 10 mothers of adolescents with DM1 who underwent treatment for DM1 with CSII. Its analysis was performed by Bardin's content analysis method. Results: from the analysis of the interviews, four categories emerged: The transition to CSII, Adolescent and family well-being, Adolescent autonomy and Limitations of the device and respective subcategories that expose the perception of mothers of adolescents with DM1 about treatment with CSII. Conclusion: treatment with CSII provides a better quality of life for adolescents and their families and favors the autonomy of adolescents. The adaptation process poses challenges and the support of health professionals is essential. Some limitations were identified in the device that hopefully will be overcome by the current technological innovations.

              Being mothers and fathers of a child with type 1 diabetes aged 1 to 7 years: a phenomenological study of parents’ experiences

              ABSTRACT Purpose: The diagnosis of diabetes in pre-school children poses a number of unique challenges related to everyday responsibility, and the continuous need for supervision and caregiving. This may affect both the child’s and the parents’ perceived burden caused by the condition. The aim of the study was to explore the lived experience of being mothers and fathers of a child with type 1 diabetes aged 1 to 7 years. Methods: The study is rooted in an interpretive phenomenological methodology as described by van Manen. In-depth interviews were carried out to collect data. Findings. We were able to identify one essential theme across the interviews: Striving to live an ordinary family life, yet feeling and living very differently—with interrelated sub-themes: A life-changing situation, Always on guard, and Struggling to let go. Conclusion: Parents described a profoundly changed situation, and they were indeed striving to live like a “normal” family. They were in need of support from health care professionals at the outpatient clinic, not only support and supervision in regard to practical tasks, but also concerning handling a changed life situation and emotional reactions, especially in the first year after diagnosis.
