      Factors associated with functional incapacity in elders living in long stay institutions in Brazil: a cross-sectional study

          Abstract

          Background

          The increase of the elderly population and the high prevalence of chronic diseases have contributed to the increasing importance of functional ability as a global public health problem. This study aimed to assess functional capacity in institutionalized elders, as well as undertake an exploratory analysis of its associated factors.

          Methods

          This is a cross-sectional study with institutionalized Brazilian elders. Functional capacity was assessed using the Katz Index for Activities of Daily Living (ADL) and the Lawton Scale for Instrumental Activities of Daily Living (IADL). The characteristics of dependent individuals were described and logistic regression models were developed for both scales. Multiple models that included all selected variables were developed using a hierarchical approach. We considered the results from the Wald test (p < 0.05) as a rule for progressing to the next level.

          Results

          A population of 760 elders was considered. The prevalence of dependence was 50.3% for ADL and 81.2% for IADL. We observed associations between ADL dependence and the following factors: self-report of stroke, difficulty of walking 400 meters, lower total scores in questions related to the temporal orientation section of the cognition test, and self-reports of frequently feeling upset. IADL dependence was associated with educational level, self-report of cancer, difficulty of walking 400 meters, use of glasses, and self-reported memory problems.

          Conclusions

          Sociodemographic and health conditions were associated with functional incapacity in institutionalized elders. Based on these findings, we emphasize the importance of both prevention and treatment of chronic conditions as well as social support in the maintenance of individuals’ autonomy.

          Mini-Exame do Estado Mental: características psicométricas em idosos ambulatoriais

          OBJETIVO: Avaliar as características de medida do Mini-Exame do Estado Mental em idosos atendidos em um ambulatório geral. MÉTODOS: O total de 303 indivíduos (>65 anos) foi submetido à avaliação geriátrica com vários instrumentos, inclusive o Mini-Exame do Estado Mental. Foram calculadas a sensibilidade, a especificidade, os valores preditivos positivo e negativo e a curva ROC. RESULTADOS: A sensibilidade, a especificidade, os valores preditivos positivo e negativo e a área sob a curva ROC foram 80,8%, 65,3%, 44,7%, 90,7% e 0,807, respectivamente (ponto de corte 23/24). O melhor ponto de corte para indivíduos analfabetos foi 18/19 (sensibilidade =73,5%; especificidade =73,9%), e para aqueles com instrução escolar foi 24/25 (sensibilidade =75%; especificidade =69,7%). CONCLUSÕES: Para o rastreamento cognitivo de idosos atendidos em ambulatórios gerais pelo Mini-Exame do Estado Mental, a escolaridade deverá ser considerada para a adoção do ponto de corte mais adequado.

            Incidence and risk factors of disability in the elderly: the Rotterdam Study.

            This study analyzed the incidence of disability and its risk factors in multiple dimensions in community-dwelling women and men of older age, between 1990 and 1999, in Rotterdam, The Netherlands. For this community-based prospective longitudinal study, data were obtained from the Rotterdam Study that comprised a cohort of 7983 elderly who are 55 and over. The study sample for incident disability consisted of 4258 subjects who were disability free at baseline and had complete outcome data at follow-up, 6 years later. Sociodemographic factors, lifestyle variables, health conditions and disability status were assessed at baseline and follow-up. Disability was defined as a Disability Index (DI) > or =0.50 according to the Health Assessment Questionnaire. Multivariate analyses, performed separately due to gender differences, revealed that age, self-rated health, overweight, depression, joint complaints, medication use were predictors of disability for both men and women. Stroke, falling and presence of comorbidities predicted disability in men only while having a partner, poor cognitive functioning, osteoarthritis and morning stiffness only predicted disability in women. Identified risk factors in this study are to some extent modifiable, enabling interventive strategies, reckoning with gender differences in risk profile, in order to prevent disability.

              Capacidade funcional e fatores associados em idosos do Sul do Brasil: um estudo de base populacional

              Estimar a prevalência de capacidade funcional inadequada e fatores associados na população idosa de Joaçaba, Santa Catarina, Brasil, entre 2003 e 2004. Realizou-se um inquérito populacional domiciliar (n = 345). Foram considerados com capacidade funcional inadequada indivíduos com dificuldade na realização de seis atividades, ou inabilidade para realizar três da escala proposta por Rikli & Jones. Foi aplicado um questionário contendo questões sobre variáveis sócio-econômicas, demográficas, tipo de moradia e percepção da própria situação econômica. Realizou-se a distribuição de freqüência simples das variáveis de interesse, teste de associação do qui-quadrado e análise de regressão logística múltipla não condicional. A taxa de resposta foi de 92,7%, prevalência de capacidade funcional inadequada de 37,1% (IC95%: 32,0-42,2). A idade média foi de 69,5 anos. Após ajustamento para fatores de confusão, permaneceram associadas à capacidade funcional inadequada ter 70 anos ou mais, ser do sexo feminino e apresentar autopercepção negativa de sua situação econômica. Muitos dos idosos de Joaçaba apresentam capacidade funcional inadequada, levando à dependência. Programas de saúde que retardem o aparecimento de incapacidades, bem como a realização de atividades sociais poderiam contribuir para um envelhecimento saudável.
