      The Influence of Class II Division 2 Malocclusions on the Harmony of the Human Face Profile

          Abstract

          Background

          Persons with class II division 2 malocclusion are characterized by a very specific dento-skeletal and soft-tissue profile (a profile in which a protruding nose and chin, retruding lips, concave and shortened lower third of the face, and gummy smile are dominant), which is the opposite of the currently modern profiles (convex profile of protruding lips and small chin). The aim of this research was to determine the differences in parameters of harmonies of facial profiles between persons with class II division 2 malocclusions and class I, and to establish the significance of those differences.

          Material/Methods

          For this study, 50 patients with class II division 2 malocclusions and 50 patients with class I were selected; profile photos were recorded and a photometric analysis was done: a type of profile according to Schwarz, the shape of a nose, the prominence of chin, biometrical field, the position of lips in relation to the tangent Sn-Pg, S-line (Steiner), E-line (Riketts) and a facial angle according to Arnett.

          Results

          The significant differences in profiles of persons with class II division 2 compared to class I were: position and prominence of the chin, the position of the lower and upper lip in relation to the S-line, and smaller value of a facial angle in relation to persons with class I.

          Conclusions

          The differences seen in skeletal profiles were not associated with significant differences in the profiled facial contours of the examined groups. The compensatory role of the fullness of soft tissues of the lips is probably the reason why there were not significant deviations in all the examined parameters.

          Skeletofacial morphology of attractive and nonattractive faces.

          The aim of this study was to answer the question: Is facial beauty related to specific skeletofacial morphology? Thirty attractive (25 females, 5 males) and 32 nonattractive (11 females, 21 males) subjects were compared. Facial attractiveness was assessed by the aid of en-face facial photographs. Skeletofacial morphology was determined using lateral head films. The radiographs were analyzed with respect to sagittal and vertical jaw relationships, facial height, profile convexity, and lip position. The relationship between the skeletofacial variables and "divine" facial proportions was evaluated with a facial disproportion index in the transverse and vertical plane. When comparing attractive with nonattractive females, the attractive females had a larger ANB angle and Wits-appraisal (P < .05 and P < .001, respectively), the soft tissue profile was more convex (P < .01), and the distances of the upper and lower lips to the "Esthetic Line" (E-line) were smaller (P < .01). When comparing nonattractive females with nonattractive males, the males had a larger Wits-appraisal (P < .01), the soft tissue profile was more convex (P < .01), and the distances of the upper and lower lips to the E-line were smaller (P < .01). A significant correlation (P < .05) between the skeletofacial variables and the transverse and vertical facial disproportion indices was found only for the ML/NL angle (transverse: r = -0.73, and vertical: r = 0.68) and for the posterior facial height (transverse: r = 0.80). It could be concluded that facial beauty in frontal view is related only to a minor degree to specific skeletofacial morphology in lateral view.

            Evaluation of Facial Beauty Using Anthropometric Proportions

            The improvement of a patient's facial appearance is one of the main goals of contemporary orthodontic treatment. The aim of this investigation was to evaluate the difference in facial proportions between attractive and anonymous females in order to establish objective facial features which are widely considered as beautiful. The study included two groups: first group consisted of 83 Caucasian female subjects between 22 and 28 years of age who were selected from the population of students at the University of Belgrade, and the second group included 24 attractive celebrity Caucasian females. The en face facial photographs were taken in natural head position (NHP). Numerous parameters were recorded on these photographs, in order to establish facial symmetry and correlation with the ideal set of proportions. This study showed significant difference between anonymous and attractive females. Attractive females showed smaller face in general and uniformity of the facial thirds and fifths, and most of the facial parameters meet the criteria of the ideal proportions.

              The use of anthropometric proportion indices in the measurement of facial attractiveness.

              This study used anthropometric data in the form of Farkas' proportion indices in order to quantify facial attractiveness, and to relate measured change through surgery, to clinical judgement. Standardized photographs of 15 orthognathic patients were used in album form and rated by 10 experienced clinicians: album 1 for facial attractiveness (before surgery) and album 2 for improvement in facial attractiveness (before and after surgery). Twenty-five proportion indices were selected and linear measurements recorded from the pre- and post-surgical photographs. The corresponding change in indices and in clinicians' scores were compared. The clinicians' assessment of the degree of improvement in facial appearance achieved through surgery, related closely to the scores produced by the change in proportion indices (r = 0.698, P = 0.004). Clinical assessment demonstrated a clear inverse relationship between initial attractiveness rating and the degree of improvement achieved through orthognathic surgery (r = -0.781, P = 0.001). The results showed good repeatability in terms of clinical assessment, photography and digitization. The method would appear to have potential for further development, possibly into a 'facial attractiveness index' for the objectives of quantification of improvement achieved through treatment.
