          Abstract

          OBJETIVO: Analisar a eficácia do tratamento com Botox® e estudar o comportamento destes pacientes após aplicações sucessivas, dando ênfase ao possível efeito de tolerância após o uso prolongado deste medicamento. MÉTODOS: Foi realizado estudo prospectivo em 16 pacientes com distonias faciais no ambulatório de Oftalmologia, no Setor de Plástica Ocular do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, entre abril de 1998 a março de 1999. Todos os pacientes foram submetidos ao exame oftalmológico completo, neurológico e tomografia computadorizada de crânio. Os pacientes com sintomas importantes de espasmo foram tratados com aplicação da toxina botulínica tipo A (Botox®). Dez pacientes eram do sexo feminino. A idade média foi 64,75 anos. RESULTADOS: Dentre as distonias faciais, o espasmo hemifacial foi o mais encontrado, num total de 8 pacientes.O índice de sucesso do Botox®foi de 87,5 %, com duração média do efeito de 30 a 90 dias, variando de acordo com o número de aplicações. CONCLUSÕES: O tratamento dos espasmos faciais com a toxina botulínica mostrou-se eficaz em 87,5 % de nossos pacientes.

          Translated abstract

          PURPOSE: To analyze the effectiveness of the treatment with Botox® and to study the behavior of these patients after successive applications, giving attention to the possible effect of tolerance after the persistent use of this medication. METHODS: A prospective study of 16 patient with facial dystonia, was performed in the Ophthalmic Plastic Department, of the "Hospital das Clínicas" of São Paulo University, from April l 998 to March 1999. All patients were submitted to ophthalmic examination, neurological examination and computerized tomography scan. The patients with important symptoms of spasm were treated with botulinum A toxin (Botox®). Ten patients were female. The mean age was 64.75 years. RESULTS: Hemifacial spasm (8 patients) was the most common form of facial dystonia. Success rate was 87.5%, the mean duration of the effect after the treatment was between 30 and 90 days. CONCLUSION: Treatment of facial spasm with botulinum A toxin was effective in 87.5% of our patients.

            Tratamento do espasmo facial unilateral com toxina botulínica tipo A

            RESUMO Este estudo retrospectivo apresenta a experiência no tratamento do espasmo facial unilateral com toxina botulínica tipo A. Dezenove pacientes receberam 71 aplicações. O índice de sucesso foi de 94,4%. A duração média do efeito foi de 17,7 (± 7,2) semanas. A incidência de complicações foi de 35,2% e dose dependente, todas elas locais, transitórias e de grau leve a moderado.

              Facial dystonia, essential blepharospasm and hemifacial spasm.

              Movement disorders, or dyskinesias, in the facial region may be categorized in several ways. Dystonic movement disorders in the cranial-cervical region, including essential blepharospasm, Meige syndrome and spasmodic torticollis, are characterized by uncontrollable squeezing movements in the face and neck. These disorders typically present in the fifth and sixth decades of life. Essential blepharospasm is particularly debilitating, as the involuntary eyelid closure that accompanies this condition may result in functional blindness with an otherwise normal visual pathway. Hemifacial spasm is an intermittent, unilateral, spasmodic contraction of the muscles innervated by the facial nerve. This disorder usually presents in the third or fourth decade and has a different underlying pathophysiology than the dystonias. Botulinum A toxin therapy has largely supplanted surgical intervention in the treatment of essential blepharospasm and hemifacial spasm.
