      No Evidence for Threat-Induced Spatial Prioritization of Somatosensory Stimulation during Pain Control Using a Synchrony Judgment Paradigm

          Abstract

          Topical research efforts on attention to pain often take a critical look at the modulatory role of top-down factors. For instance, it has been shown that the fearful expectation of pain at a location of the body directs attention towards that body part. In addition, motivated attempts to control this pain were found to modulate this prioritization effect. Such studies have often used a temporal order judgment task, requiring participants to judge the order in which two stimuli are presented by indicating which one they perceived first. As this constitutes a forced-choice response format, such studies may be subject to response bias. The aim of the current study was to address this concern. We used a ternary synchrony judgment paradigm, in which participants judged the order in which two somatosensory stimuli occurred. Critically, participants now also had the option to give a ‘simultaneous’ response when they did not perceive a difference. This way we eliminated the need for guessing, and thus reduced the risk of response bias. One location was threatened with the possibility of pain in half of the trials, as predicted by an auditory cue. Additionally, half of the participants (pain control group) were encouraged to avoid pain stimuli by executing a quick button press. The other half (comparison group) performed a similar action, albeit unrelated to the occurrence of pain. Our data did not support threat-induced spatial prioritization, nor did we find evidence that pain control attempts influenced attention in any way.

            Oxytocin-Motivated Ally Selection is Moderated by Fetal Testosterone Exposure and Empathic Concern

            In humans, the hypothalamic neuropeptide oxytocin shifts the individual’s focus on self-interest toward group-serving cognitions and decision-making. Here we examine this general tendency in the context of group formation, where individuals included into their group (or not) 18 targets morphed as having low or high-threat potential (with high-threat targets being beneficial to group-interests but potentially hurting the recruiter’s self-interest). Ninety healthy males self-administered oxytocin or placebo in a randomized double-blind, placebo-controlled study design, had their hands scanned to derive fetal testosterone vs. estradiol exposure from their 2D:4D ratio, and self-reported on their chronic empathic concern. Multilevel regression models revealed that when given oxytocin rather than placebo, individuals with low fetal testosterone priming included low-threat targets more and high-threat targets (somewhat) less. Individuals with high fetal testosterone (i.e., low estradiol) exposure, however, included high-threat targets more, and low-threat targets less when given oxytocin rather than placebo. Second, when given oxytocin rather than placebo, individuals with low empathic concern included low-threat targets more and high-threat targets less. Individuals with high empathic concern, however, included high-threat targets more, and low-threat targets less when given oxytocin rather than placebo. We conclude that oxytocin shifts the individual’s focus from self to group-serving cognition and decision-making, and that these tendencies are stronger for males with high rather than low fetal testosterone vs. estradiol exposure, and high rather than low empathic concern. Implications and avenues for future research are discussed.

              Keeping pain in mind: a motivational account of attention to pain.

              Attention is a key concept in many theories of pain perception. A clinically popular idea is that pain is more intense in persons who are hypervigilant for or bias their attention to pain information. So far, evidence for such bias in pain patients as compared to healthy persons is inconclusive. Furthermore, studies investigating the effects of distracting attention away from pain have shown contradictory results. In this review, we present a motivational perspective on attentional processing of pain that accounts for these inconclusive research findings. We argue that pain always has to be considered within a context of goal pursuit. From this perspective, two largely unexplored theoretical assumptions are introduced. First, when pain occurs during the pursuit of a certain goal, it may unintentionally capture attention although it is not relevant for the goal. Whether such unintentional attentional capture happens is not only dependent upon the characteristics of the pain but also on the characteristics of the focal goal. Second, attention to pain and pain-related information might be driven by a focal goal related to pain. Attentional processing of pain information will be particularly enhanced when the focal goal is related to pain management (e.g., attempting to gain control). Future research should systematically investigate the role of motivation and goal pursuit in the attentional processing of pain-related information. This motivational perspective offers a powerful framework to explain inter- and intra-individual differences in the deployment of attention to pain-related information.
