          Abstract

          OBJETIVO: o estudo busca determinar a existência de associação entre a elevação da pressão arterial e os níveis plasmáticos dos peptídeos natriuréticos ANP e BNP, na gestação complicada pela pré-eclâmpsia. MÉTODOS: em estudo transversal caso-controle, pareado por idade gestacional, 25 grávidas normotensas e 61 portadoras de pré-eclâmpsia foram avaliadas quanto ao nível da pressão arterial e às concentrações plasmáticas dos peptídeos natriuréticos ANP e BNP. Exames clínico e laboratoriais foram realizados para o diagnóstico de pré-eclâmpsia, sendo a pressão arterial medida de forma não invasiva. As dosagens hormonais foram obtidas por radioimunoensaio, após extração em colunas Sep-pak C18. Os valores médios das concentrações plasmáticas do ANP e BNP foram comparados entre grupos com pressão arterial progressivamente maiores. A correlação entre os valores da pressão arterial e os níveis plasmáticos do ANP e BNP no sangue materno foi também investigada pela de análise de regressão no grupo completo de gestantes e em grupos específicos excluindo-se a hipertensão anterior à gestação e, em seguida, excluindo-se aquelas sem hipertensão prévia. RESULTADOS: os valores plasmáticos de ANP foram 41.5±7.3, 78.4±13.1 e 89.2±13.4 pg/mL (p<0,00001) e os de BNP plasmático foram 79.5±15.8, 176.7±42.2 e 208.3±63.5 pg/mL (p=0,005), respectivamente, para os grupos de pressão arterial média =107 mmHg, 107-139 mmHg e =140 mmHg. Verificou-se correlação positiva entre as concentrações plasmáticas do ANP e os níveis pressóricos na pré-eclâmpsia, independente da existência de estado hipertensivo prévio à gestação (p<0,0001 para pré-eclâmpsia e p<0,01 para pré-eclampsia sobreposta à hipertensão arterial crônica), ao passo que as dosagens de BNP não se mostraram associadas à pressão arterial no grupo com hipertensão arterial prévia à gestação (p=0,004 para pré-eclâmpsia e p=0,18 para pré-eclampsia sobreposta à hipertensão arterial crônica). CONCLUSÃO: o agravamento da hipertensão na pré-eclâmpsia correlacionou-se com as concentrações séricas do ANP e BNP, embora os valores do BNP possam ser influenciados pela existência de estado hipertensivo prévio.

          Translated abstract

          PURPOSE: to determine the existence of association between blood pressure rise and plasma ANP and BNP levels in pregnancies complicated by preeclampsia, considering the existence of a hypertensive state before pregnancy and supportive drug influence on these hormones. METHODS: in a case-control transversal study, 86 pregnant women were assessed regarding arterial pressure level and plasma ANP and BNP levels. Clinical and laboratory tests were carried out to diagnose preeclampsia and the use of hypotensive drugs and magnesium sulfate was considered. Hormone determinations were obtained through radioimmunoassay, after extraction in C18 Sep-pak columns. Correlation was investigated by means and regression analysis in the whole group of pregnant women and in specific groups, considering prior hypertension. RESULTS: plasma ANP values were 41.5±7.3, 78.4±13.1 and 89.2±13.4pg/mL (p<0.00001) and plasma BNP values were 79.5±15.8, 176.7±42.2 and 208.3±63.5 pg/mL (p=0.005), respectively, for mean blood pressure =107 mmHg, 107-139 mmHg and =140 mmHg. It was verified that the positive correlation between plasma ANP concentrations and pressure levels in preeclampsia did not depend on the existence of a hypertensive state before pregnancy (p<0.0001: preeclampsia and p<0.01: preeclampsia superimposed on chronic hypertension), whereas BNP dosages were not associated with the arterial pressure in the group with arterial hypertension prior to pregnancy (p=0.004: preeclampsia and p=0.18: preeclampsia superimposed on chronic hypertension). CONCLUSION: aggravation of hypertension in preeclampsia correlates with serum ANP and BNP concentrations, although BNP values may be influenced by the existence of a prior hypertensive state.

          Brain natriuretic peptide as a novel cardiac hormone in humans. Evidence for an exquisite dual natriuretic peptide system, atrial natriuretic peptide and brain natriuretic peptide.

          Using a specific radioimmunoassay for human brain natriuretic peptide (hBNP) with a monoclonal antibody, we have investigated its synthesis, secretion, and clearance in comparison with those of atrial natriuretic peptide (ANP) in normal subjects and patients with congestive heart failure (CHF). Mean BNP-like immunoreactivity (-LI) levels in normal atrium and ventricle were 250 and 18 pmol/g, respectively. The plasma BNP-LI level in normal subjects was 0.90 +/- 0.07 fmol/ml, which was 16% of the ANP-LI level. In contrast, the plasma BNP-LI level markedly increased in patients with CHF in proportion to its severity, and surpassed the ANP-LI level in severe cases. There was a significant step-up of the plasma BNP-LI level in the coronary sinus (CS) compared with that in the aortic root (Ao) and the difference between these BNP-LI levels, delta(CS-Ao)BNP, also increased with the severity of CHF. In addition, the step-up of the BNP-LI level in the anterior interventricular vein [delta(AIV-Ao)BNP] was comparable to delta(CS-Ao)BNP, indicating that BNP is secreted mainly from the ventricle. Predominant BNP synthesis in the ventricle was also confirmed by Northern blot analysis. Catheterization and pharmacokinetic studies revealed that hBNP is cleared from the circulation more slowly than alpha-hANP; this was in part attributed to lower (about 7%) binding affinity of hBNP to clearance receptors than that of alpha-hANP. A predominant molecular form of BNP-LI in the heart and plasma was a 3-kD form corresponding to hBNP. These results indicate that BNP is a novel cardiac hormone secreted predominantly from the ventricle, and that the synthesis, secretion and clearance of BNP differ from those of ANP, suggesting discrete physiological and pathophysiological roles of BNP in a dual natriuretic peptide system.

            A new natriuretic peptide in porcine brain.

            Atrial natriuretic peptide (ANP), a hormone secreted from mammalian atria, regulates the homoeostatic balance of body fluid and blood pressure. ANP-like immunoreactivity is also present in the brain, suggesting that the peptide functions as a neuropeptide. We report here identification in porcine brain of a novel peptide of 26 amino-acid residues, eliciting a pharmacological spectrum very similar to that of ANP, such as natriuretic-diuretic, hypotensive and chick rectum relaxant activities. The complete amino-acid sequence determined for the peptide is remarkably similar to but definitely distinct from the known sequence of ANP, indicating that the genes for the two are distinct. Thus, we have designated the peptide 'brain natriuretic peptide' (BNP). The occurrence of BNP with ANP in mammalian brain suggests the possibility that the physiological functions so far thought to be mediated by ANP may be regulated through a dual mechanism involving both ANP and BNP.

              Report of the National High Blood Pressure Education Program Working Group on High Blood Pressure in Pregnancy.

              This report updates the 1990 "National High Blood Pressure Education Program Working Group Report on High Blood Pressure in Pregnancy" and focuses on classification, pathophysiologic features, and management of the hypertensive disorders of pregnancy. Through a combination of evidence-based medicine and consensus this report updates contemporary approaches to hypertension control during pregnancy by expanding on recommendations made in "The Sixth Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure." The recommendations to use Korotkoff phase V for determination of diastolic pressure and to eliminate edema as a criterion for diagnosing preeclampsia are discussed. In addition, the use as a diagnostic criterion of blood pressure increases of 30 mm Hg systolic or 15 mm Hg diastolic with blood pressure <140/90 mm Hg has not been recommended, because available evidence shows that women with blood pressures fitting this description are not more likely to have adverse outcomes. Management distinctions are made between chronic hypertension that is present before pregnancy and hypertension that occurs as part of the pregnancy-specific condition of preeclampsia, as well as management considerations for women with comorbid conditions. A discussion of the pharmacologic treatment of hypertension during pregnancy includes recommendations for specific agents. The use of low-dose aspirin, calcium, or other dietary supplements in the prevention of preeclampsia is described, and expanded sections on counseling women for future pregnancies and recommendations for future research are included.
