      Chrono-nutrition: a review of current evidence from observational studies on global trends in time-of-day of energy intake and its association with obesity

          Abstract

          The importance of the circadian rhythm in regulating human food intake behaviour and metabolism has long been recognised. However, little is known as to how energy intake is distributed over the day in existing populations, and its potential association with obesity. The present review describes global trends in time-of-day of energy intake in the general population based on data from cross-sectional surveys and longitudinal cohorts. Evidence of the association between time-of-day of energy intake and obesity is also summarised. Overall, there were a limited number of cross-sectional surveys and longitudinal cohorts that provided data on time-of-day of energy intake. In the identified studies, a wide variation in time-of-day of energy intake was observed, with patterns of energy distribution varying greatly by country and geographical area. In relation to obesity, eight cross-sectional surveys and two longitudinal cohorts were identified. The association between time-of-day of energy intake and obesity varied widely, with several studies reporting a positive link between evening energy intake and obesity. In conclusion, the current review summarises global trends in time-of-day of energy intake. The large variations across countries and global regions could have important implications to health, emphasising the need to understand the socio-environmental factors guiding such differences in eating patterns. Evidence of the association between time-of-day of energy intake and BMI also varied. Further larger scale collaborations between various countries and regions are needed to sum data from existing surveys and cohorts, and guide our understanding of the role of chrono-nutrition in health.

          High caloric intake at breakfast vs. dinner differentially influences weight loss of overweight and obese women.

          Few studies examined the association between time-of-day of nutrient intake and the metabolic syndrome. Our goal was to compare a weight loss diet with high caloric intake during breakfast to an isocaloric diet with high caloric intake at dinner. Overweight and obese women (BMI 32.4 ± 1.8 kg/m(2) ) with metabolic syndrome were randomized into two isocaloric (~1400 kcal) weight loss groups, a breakfast (BF) (700 kcal breakfast, 500 kcal lunch, 200 kcal dinner) or a dinner (D) group (200 kcal breakfast, 500 kcal lunch, 700 kcal dinner) for 12 weeks. The BF group showed greater weight loss and waist circumference reduction. Although fasting glucose, insulin, and ghrelin were reduced in both groups, fasting glucose, insulin, and HOMA-IR decreased significantly to a greater extent in the BF group. Mean triglyceride levels decreased by 33.6% in the BF group, but increased by 14.6% in the D group. Oral glucose tolerance test led to a greater decrease of glucose and insulin in the BF group. In response to meal challenges, the overall daily glucose, insulin, ghrelin, and mean hunger scores were significantly lower, whereas mean satiety scores were significantly higher in the BF group. High-calorie breakfast with reduced intake at dinner is beneficial and might be a useful alternative for the management of obesity and metabolic syndrome. Copyright © 2013 The Obesity Society.

            Timing of energy intake during the day is associated with the risk of obesity in adults.

            The timing of energy intake is a modifiable behaviour that may influence energy regulation and the risk of obesity. We examined the associations of energy intake in the morning, midday and evening with body mass index (BMI) (n = 239). Cross-sectional analyses were performed using data from the University of California, Los Angeles Energetics Study. Energy intake was assessed using three 24-h dietary recalls and stratified by time-of-day: morning (00.00 h to 11.00 h), midday (11.00 h to 17.00 h) and evening (17.00 h to 00.00 h). Sensitivity analysis was conducted among 'true-reporters', whose self-reported energy intake was ±25% of total energy expenditure measured by doubly-labelled water (n = 99). Logistic regression models were performed adjusting for age, sex, race, education, total daily energy intake and physical activity. Energy intake in the morning was not associated with BMI. Participants who consumed ≥33% (versus <33%) of their daily energy intake at 12.00 h were (nonsignificantly) less likely to be overweight/obese [odds ratio (OR) = 0.68; 95% confidence interval (CI) = 0.37-1.24] and this association was stronger and statistically significant among true-reporters (OR = 0.34; 95% CI = 0.12-0.95). Those who consumed ≥33% of daily energy intake in the evening were two-fold more likely overweight/obese (OR = 2.00; 95% CI = 1.03-3.89), although this association was not significant among true-reporters (OR = 2.10; 95% CI = 0.60-7.29). These data indicate that eating more of the day's total energy intake at midday is associated with a lower risk of being overweight/obese, whereas consuming more in the evening is associated with a higher risk. Randomised trials are needed to test whether shifting energy intake earlier in the day could have a regulatory effect with respect to reducing intake in the evening, thereby promoting weight loss and maintenance. © 2013 The British Dietetic Association Ltd.

              Snacking increased among U.S. adults between 1977 and 2006.

              This study built on limited knowledge about patterns and trends of adult snacking in the US. We selected adults aged 19 y and older (n = 44,754) between 1977-1978 and 2003-2006 with results weighted and adjusted for sample design effects. Differences testing, by a Student's t test, used STATA 10 (P < or = 0.01). We defined a snacking event as intake of foods over a 15-min period and excluded food defined as snacks but eaten at a meal. Dietary data were obtained from the first 2 d for the 1977-1978 Nationwide Food Consumption Survey (NFCS 77) and the 1989-1991 Continuing Survey of Food Intake by Individuals (CSFII 89); and 2-d dietary data from the 1994-1996 CSFII (CSFII 96) and the NHANES from 2 consecutive surveys: NHANES 2003-2004 and NHANES 2005-2006 (NHANES 03-06). Results showed that snacking prevalence increased significantly from 71 to 97% in 2003-2006 with increases in both the 1989-1994 and the 1994-2006 periods. In all adults, snacking occasions increased 0.97 events over this time period (P < 0.01) and the contribution of snacks to total energy intake increased from 18 to 24% (P < 0.01). The energy density of snacks (food plus beverages) also increased progressively over the time period studied. Important changes in snacking food sources were found among desserts, salty snacks, candies, and sweetened beverages. More research is needed to gain a better understanding of the implications for overall energy intake and energy imbalance.
