      Determination of copro-prevalence of Echinococcus granulosus and associated factors in domestic dogs: a household cross-sectional study in Huancarama, Peru

          Abstract

          ABSTRACT Echinococcosis is an important disease with regard to public health and the leading role that humans have in fulfilling the transmission cycle. The objective of this study was to determine the copro-prevalence of Echinococcus granulosus in dogs from homes in Huancarama, Peru, and the factors associated with this infection. The research was basic, prospective, quantitative, observational, cross-sectional, and analytical. This study was approved by the Institutional Ethics Committee for the Use of Animals (CIEA) and the Institutional Research Ethics Committee (CIEI) of the Universidad Peruana Cayetano Heredia. The sample comprised of 519 homes. A geographic information system (GIS) was used to develop layers of information on the study area with georeferencing of the locations of these homes. Information processing was performed using Excel for Windows 2010, Statistical Package for the Social Sciences SPSS 25 software, and ArcGIS 10.8. Univariate and multivariate logistic regression tests were performed to determine the possible associations. Categorical variables were statistically contrasted using the chi-square test with 95% confidence intervals and P ≤ 0.05, which indicated the extreme degree of significance. It was found that 94.4% of the houses had dogs and that the prevalence of E. granulosus was 27.7% (95/343; 95% CI 22.8-32.6). The distance from the house to the cattle slaughterhouse was associated with disease occurrence (P < 0.01). Locations in the Suni altitude zone presented a higher prevalence (41.8%) (P < 0.05). Lack of knowledge that humans can contract echinococcosis was associated with disease occurrence (P < 0.05).

          Demography of domestic dogs in rural and urban areas of the Coquimbo region of Chile and implications for disease transmission.

          A cross-sectional household questionnaire survey was conducted along two transects (80 and 45km long) from Coquimbo and Ovalle cities to the Fray Jorge National Park (FJNP) in the Coquimbo region of Chile in 2005-2007 to investigate the demography of dogs in the context of a study of canine infectious diseases. Data were collected on the number of dogs per household, fecundity, mortality, and sex and age distribution. The results from 1021 households indicated that dog ownership was common, with a higher proportion of households owning dogs in rural areas (89%), than in towns (63%) or cities (49%). Dog density ranged from 1380+/-183 to 1509+/-972 dogs km(-2) in cities, from 119+/-18 to 1544+/-172 dogs km(-2) in towns, and from 1.0+/-0.4 to 15.9+/-0.4 dogs km(-2) in rural sites. The dog population was estimated to be growing at 20% in cities, 19% in towns and 9% in rural areas. The human:dog ratio ranged from 5.2 to 6.2 in cities, from 2.3 to 5.3 in towns, and from 1.1 to 2.1 in rural areas. A high percentage of owned dogs was always allowed to roam freely in the different areas (27%, 50% and 67% in cities, towns and rural areas, respectively). Observations of free-roaming dogs of unknown owner were reported from a greater proportion of respondents in cities (74%), followed by towns (51%) and finally by rural areas (21%). Overall only 3% of dogs had been castrated. In addition, only 29% of dogs were reported to have been vaccinated against canine distemper virus (CDV) and 30% against canine parvovirus (CPV). The higher population size and density, higher growth rate and a higher turnover of domestic dogs in urban than in rural areas and the poorly supervised and inadequately vaccinated dog populations in urban areas suggest that urban areas are more likely to provide suitable conditions for dogs to acts as reservoirs of pathogenic infections.             Cystic echinococcosis in South America: a call for action

            ABSTRACT Cystic echinococcosis (CE) or hydatidosis, a parasitic zoonosis caused by a cestode of the family Taeniidae, species Echinococcus granulosus, is endemic in Argentina, Chile, Peru, Uruguay, and southern Brazil. This report presents CE figures for these five countries in 2009 – 2014 and proposes indicators to measure national control programs. Nearly 5 000 new CE cases were diagnosed annually in the five countries during the study period. The average case fatality rate was 2.9%, which suggests that CE led to approximately 880 deaths in these countries during the 6-year period. CE cases that required secondary or tertiary health care had average hospital stays of 10.6 days, causing a significant burden to health systems. The proportion of new cases (15%) in children less than 15 years of age suggests ongoing transmission. Despite figures showing that CE is not under control in South America, the long-standing implementation of national and local control programs in three of the five countries has achieved reductions in some of the indicators. The Regional Initiative for the Control of CE, which includes the five countries and provides a framework for networking and collaboration, must intensify its efforts.

              Equinococosis/hidatidosis en la VII Región de Chile: diagnóstico e intervención educativa

              El presente estudio se proyectó para un trabajo simultáneo en tres ámbitos: a) diagnóstico serológico y radiológico y tratamiento quirúrgico de la hidatidosis en la población humana asintomática; b) diagnóstico animal y tratamiento de los perros, y c) evaluación de conocimientos e intervención educativa en familias campesinas y en profesionales y técnicos en salud, agropecuaria y educación, con el fin de contribuir al control del ciclo de transmisión de la enfermedad. Se efectuaron pruebas de hemaglutinación indirecta y ELISA a 5 566 personas aparentemente sanas. Los 42 (0,8%) casos con resultados positivos en ambas pruebas (seroprevalencia de 754,6 por 100 000) fueron citados para ser sometidos a una ecografía hepática y una radiografía de tórax y, de los 26 que acudieron, 16 presentaron imágenes compatibles con quiste hidatídico. Estos 16 casos fueron enviados al hospital para ser intervenidos y en los 9 que acudieron se confirmó el diagnóstico quirúrgicamente, lo cual representa una prevalencia de 161,7 por 100 000. En 2 358 perros se procedió a la detección de la forma estrobilar de Echinococcus granulosus mediante purga con bromhidrato de arecolina y se obtuvieron resultados positivos en 11%. Los datos oficiales registrados en los mataderos revelaron la presencia de quistes hidatídicos en 13% de los bovinos, 4,4% de los ovinos y 4,2% de los porcinos sacrificados en la región. El programa educativo comprendió una evaluación de conocimientos mediante una encuesta al cabeza de familia, la intervención educativa entre las familias por un proceso participatorio activo no formal de carácter lúdico en el que participaron 1 082 familias, y la intervención educativa entre profesionales y técnicos mediante metodología a distancia y presencial. Para evaluar los resultados del programa se compararon los resultados de las pruebas de conocimientos antes y después de la intervención educativa en 200 familias que participaron en ella (casos) y en 95 que no participaron (controles). Del análisis de los conocimientos sobre equinococosis/hidatidosis, 783 familias (55%) demostraron no saber nada sobre la infección. Se observó que las técnicas participatorias lúdicas se adaptan a la forma de ser del campesino y permiten obtener cambios. Se capacitaron 276 profesionales de la salud, 201 auxiliares técnicos y 453 profesores rurales. Entre los equipos de atención primaria rural, el programa tuvo una cobertura de 100%.
