      Association of Bicipital Peritendinous Effusion with Subacromial Impingement: A Dynamic Ultrasonographic Study of 337 Shoulders

          Abstract

          Bicipital peritendinous effusion (BPE) is the most common biceps tendon abnormality and can be related to various shoulder ultrasonographic findings. Since the association of BPE with subacromial impingement is unclear, our study aimed to explore its association with the dynamic subacromial impingement test during ultrasound (US) imaging. We included 337 shoulders referred for US examinations and quantified the amount of BPE. Effusion more than 1 mm in thickness was considered a positive finding. A comparison of three grades of subacromial impingement, adjusted by patient demographics, static sonographic shoulder pathology, and physical findings, by using multivariate regression models revealed that the odds ratio of subacromial impingement (with 95% confidence intervals) in the presence of BPE was 6.54 (3.21–13.32) in grade 1, 6.93 (3.05–15.76) in grade 2 and 3.18 (1.48–6.80) in grade 3. An increase in age, subdeltoid bursitis, full-thickness supraspinatus tendon tear, and shoulder stiffness were also associated with BPE. Since our study demonstrated a positive association of BPE with all grades of impingement, a US dynamic subacromial impingement test is suggested when BPE is present. Future prospective studies are needed to identify changes in BPE after treatment.

          The role of the long head of the biceps muscle and superior glenoid labrum in anterior stability of the shoulder.

          The authors conducted a study to determine if the long head of the biceps muscle and its attachment at the superior glenoid labrum play a role in stability of the shoulder in an overhead position. Their study used a dynamic cadaveric shoulder model that simulated the forces of the rotator cuff and long head of biceps muscles as the glenohumeral joint was abducted and externally rotated. Their data suggest that the long head of the biceps muscle contributes to anterior stability of the glenohumeral joint by increasing the shoulder's resistance to torsional forces in the vulnerable abducted and externally rotated position. The biceps muscle also helps to diminish the stress placed on the inferior glenohumeral ligament. Detachment of the superior glenoid labrum is detrimental to anterior shoulder stability as it decreases the shoulder's resistance to torsion and places a greater magnitude of strain on the inferior glenohumeral ligament.

            Diagnostic accuracy of ultrasound for rotator cuff tears in adults: a systematic review and meta-analysis.

            To determine the diagnostic accuracy of ultrasound to detect partial and complete thickness rotator cuff tears based on all available clinical trials. An electronic search of databases registering published and unpublished literature was conducted. All diagnostic accuracy studies that directly compared the accuracy of ultrasound (the index test) to either arthroscopic or open surgical findings (the reference test) for rotator cuff tear were included. The methodological quality of each included study was assessed using the QUADAS form. When appropriate, pooled sensitivity and specificity analysis was conducted, with an assessment of the summary receiver operating characteristic (ROC) curve for each analysis. Sixty-two studies assessing 6007 patients and 6066 shoulders were included. Ultrasonography had good sensitivity and specificity for the assessment of partial thickness (sensitivity 0.84; specificity 0.89), and full-thickness rotator cuff tears (sensitivity 0.96; specificity 0.93). However, the literature poorly described population characteristics, assessor blinding, and was based on limited sample sizes. The literature assessing transducer frequency was particularly small in size. Ultrasonography is an appropriate radiological technique for the assessment of rotator cuff tears with an acceptable sensitivity and specificity. The diagnostic test accuracy of ultrasound is superior for the detection of full-thickness compared to partial-thickness cuff tears.               EURO-MUSCULUS/USPRM Basic Scanning Protocols for shoulder.

              In this protocol, the patient/probe positionings, anatomical drawings and ultrasound images of commonly scanned shoulder structures are described. This practical guide is prepared (with an international consensus of several expert physiatrists) to serve as a uniform/standard approach especially for beginner sonographers.
