          Abstract

          The aim of this research was to identify the reasons for satisfaction and dissatisfaction among oncology nursing professionals working in Brazil and Portugal. A descriptive and qualitative study was conducted with nine Portuguese nurses and 17 Brazilian nursing professionals, using a questionnaire and interview, which were analyzed according to Bardin's proposition. Satisfaction, in both scenarios, was associated, above all, to the patient and the treatment process, and the bond established between the professional and the individual demanding care. Dissatisfaction resulted primarily from the extensive exposure to high workload and to the deaths of oncology patients. The importance of considering the subjectivity that permeates the work scenario is highlighted, which can have vast and, sometimes, costly implications.

          Objetivou-se nesta pesquisa identificar os motivos de satisfação e insatisfação entre profissionais de enfermagem que atuavam na atenção oncológica, no Brasil e em Portugal. Estudo com abordagem qualitativa, descritivo, realizado com nove enfermeiros portugueses e dezessete profissionais de enfermagem brasileiros, utilizando-se de questionário e entrevista. Os dados foram analisados na proposta de Bardin. A satisfação, em ambos os cenários, esteve associada, sobretudo, ao paciente e processo de tratamento, e ao vínculo estabelecido entre o profissional e o indivíduo que demanda por seus cuidados. A insatisfação decorreu, prioritariamente, da exposição à exaustiva carga de trabalho e óbito do paciente oncológico. Foi destacada a relevância de se atentar à subjetividade que permeia o cenário laboral, cujas implicações podem ser vastas e, por vezes, onerosas.

          El objetivo de esta investigación fue identificar las razones de la satisfacción y insatisfacción entre los profesionales de enfermería que trabajan en la atención oncológica en Brasil y Portugal. Estudio descriptivo y cualitativo realizado con nueve enfermeras portuguesas y diecisiete profesionales de la enfermería brasileña, utilizando cuestionarios y entrevistas, más tarde una transcripción y analizado con la proposición de Bardin. La satisfacción, en ambos escenarios, se asocia, sobre todo, para el paciente y el proceso de tratamiento, y el vínculo que se establece entre el profesional y la persona exigente para su cuidado. La insatisfacción se debió principalmente a la amplia exposición de la carga de trabajo y la muerte de los pacientes oncológicos. Destacó la importancia de considerar la subjetividad que impregna el escenario de trabajo, que puede tener enorme y, a veces, consecuencias costosas.

            Effects of extended work shifts on employee fatigue, health, satisfaction, work/family balance, and patient safety.

            12-hour shifts are quickly spreading in Europe. From our multivariate analysis concerning 25,924 European nurses, including twenty explanatory variables simultaneously, we found that work schedule itself is not a major determinant factor. Nurses aim to choose or accept night shifts or 12-hour shift in order to reduce their work/home conflicts, however, at the expense of the patient's safety, as well as their own health and safety. Therefore, it is important to develop measures, such as extended child care, association of nurses to the elaboration of their rota, 9- or 10-hour shifts in the afternoon, allowing naps during night shifts, and reduction of changing shifts with short notice. Work schedules must be organized in order to allow time for shift handover, social support and team building.

              Should I stay or should I go? The impact of working time and wages on retention in the health workforce

              Background Turnover in the health workforce is a concern as it is costly and detrimental to organizational performance and quality of care. Most studies have focused on the influence of individual and organizational factors on an employee’s intention to quit. Inspired by the observation that providing care is based on the duration of practices, tasks and processes (issues of time) rather than exchange values (wages), this paper focuses on the influence of working-time characteristics and wages on an employee’s intention to stay. Methods Using data from the WageIndicator web survey (N = 5,323), three logistic regression models were used to estimate health care employee’s intention to stay for Belgium, Germany and the Netherlands. The first model includes working-time characteristics controlling for a set of sociodemographic variables, job categories, promotion and organization-related characteristics. The second model tests the impact of wage-related characteristics. The third model includes both working-time- and wage-related aspects. Results Model 1 reveals that working-time-related factors significantly affect intention to stay across all countries. In particular, working part-time hours, overtime and a long commuting time decrease the intention to stay with the same employer. The analysis also shows that job dissatisfaction is a strong predictor for the intention to leave, next to being a woman, being moderately or well educated, and being promoted in the current organization. In Model 2, wage-related characteristics demonstrate that employees with a low wage or low wage satisfaction are less likely to express an intention to stay. The effect of wage satisfaction is not surprising; it confirms that besides a high wage, wage satisfaction is essential. When considering all factors in Model 3, all effects remain significant, indicating that attention to working and commuting times can complement attention to wages and wage satisfaction to increase employees’ intention to stay. These findings hold for all three countries, for a variety of health occupations. Conclusions When following a policy of wage increases, attention to the issues of working time—including overtime hours, working part-time, and commuting time—and wage satisfaction are suitable strategies in managing health workforce retention.
