      Effect of Narrative Nursing Combined with Thinking Map Health Education on Parturient Self-Efficacy and Neonatal Nursing Ability

          Abstract

          The motivation behind this study is to explore the influence of narrative nursing combined with thinking map health education on parturient self-efficacy and neonatal nursing ability. To verify this claim, we have selected 60 parturients who were treated in the hospital from February 2019 to April 2021. Moreover, these patients were randomly assigned to control and study groups. The former received narrative nursing, and the latter received narrative nursing combined with thinking map health education. The nursing satisfaction, maternal and neonatal nursing ability, maternal health knowledge awareness score, maternal self-efficacy, anxiety, and depression scores were compared. Initially, we have compared the nursing satisfaction: in the study group, 22 cases were satisfied, 8 cases were basically satisfied, and 0 cases were dissatisfied, and the satisfaction rate was 100.00%; in the control group, 12 cases were satisfied, 14 cases were basically satisfied, and 4 cases were dissatisfied, and the satisfaction rate was 86.67%. The nursing satisfaction in the study group was higher compared to the control group ( P < 0.05). Secondly, we compared the mastery of neonatal nursing ability. The study group mastered the relevant knowledge of neonatal nursing in 23 cases, basically mastered 6 cases, and did not master 1 case, with a mastery rate of 96.67%. The control group mastered the knowledge of neonatal nursing in 12 cases, basically mastered 10 cases, and did not master 8 cases with a mastery rate of 73.33%. In the comparison between the two groups, the mastery of neonatal nursing ability in the study group was higher compared to the control group ( P < 0.05). Considering the scores of maternal health knowledge, the scores of neonatal basic nursing, physiological characteristics, breastfeeding methods, environmental nursing knowledge, vaccination, and physical examination requirements in the study group were higher compared to the control group ( P < 0.05). In the comparison of maternal self-efficacy, the total scores for promoting development, health care, safety, feeding, and self-efficacy in the study group were significantly higher compared to the control group ( P < 0.05). Finally, we compared the scores for anxiety and depression. Before nursing, there exhibited no significant difference ( P > 0.05). After nursing, the anxiety and depression scores decreased. Furthermore, the anxiety and depression scores of the study group were lower compared to the control group ( P < 0.05). In the health education of primary parturient, the adoption of narrative nursing combined with thought guiding schema can enhance the level of nursing satisfaction and self-efficacy, strengthen the nursing ability of primiparas and the awareness rate of health knowledge, and reduce the occurrence of a maternal bad mood, which is more in line with the requirements of parturient.

          The combined effects of Lamaze breathing training and nursing intervention on the delivery in primipara

          Abstract Background: Lamaze breathing has been widely used as a breathing training method. Nursing intervention including postural nursing, delivery ball, doula nursing, massage and psychological nursing is usually provided by nurses during labor. A number of clinical studies have investigated the effect of Lamaze breathing training combined with nursing intervention on maternal pain relief and outcomes improvement. But there were some scholars who were against it. Methods: Randomized controlled trials from January 2000 to November 2019 in PubMed, Cochrance Library, Medline, Web of Science, Embase, Chinese Academic Journals, Chinese Biomedical Literature Database, VIP Database, Wanfang Database were searched. Two researchers independently screened the literature according to the criteria. After extracting the data, the researchers used Cochrane system to evaluate the literature quality. Statistical analyses were performed by using Comprehensive Meta Analysis V2 software. Results: Twenty-two randomized controlled trials conducted on 7035 primiparas were eligible. The results revealed that Lamaze breathing training combined with nursing intervention increased the rate of natural delivery (relative risk [RR] = 2.97, 95% confidence interval [CI] [2.48, 3.56]), shortened the length of labor (−2.604, 95% CI [−3.120, −2.087]), alleviated labor pain (RR = 0.194, 95% CI [0.115, 0.325]) and reduced postpartum bleeding (−2.966, 95% CI [−4.056, −1.877]). Conclusions: Lamaze breathing training combined with nursing intervention was effective for ameliorating the process and outcomes of childbirth in primiparae and deserves to be promoted and applied in clinical practice.

            Association Between Maternal Glucose/Lipid Metabolism Parameters and Abnormal Newborn Birth Weight in Gestational Diabetes Complicated by Preeclampsia: A Retrospective Analysis of 248 Cases

            Introduction Women with gestational diabetes mellitus (GDM) with co-existent preeclampsia (GCP) are at increased risk of giving birth to a baby with an abnormal birth weight. We have analyzed the risk factors for abnormal newborn birth weight (NBW) in women with co-presence of GDM and GCP, focusing on maternal glucose/lipid metabolism, with the aim to optimize the clinical intervention strategy. Methods The clinical data of 248 pregnant women with GCP and their infants were retrospectively analyzed through a comprehensive review of the electronic medical records of Women and Children’s Hospital, Xiamen University (Xiamen, China). These women had received prenatal care and had their baby delivered in the hospital between January 2016 and November 2018. Major characteristics assessed were large for gestational age (LGA), small for gestational age (SGA), severe preeclampsia (S-PE), and maternal plasma glucose/lipid profile in late pregnancy. Secondary characteristics were maternal age, height, body mass index (BMI), gestational weight gain (GWG), abortion history, education level, primipara or not, preterm or not, and fetal gender. Regression analysis was used to analyze the association between maternal glucose/lipid metabolism parameters and LGA or SGA. Results There was no difference in the ratio of advanced maternal age, primipara, abortion history, preterm delivery, and newborn sex between the control group and the LGA or SGA group. Logistic regression analysis, with such factors as maternal stature, BMI, among others, was applied. Multivariate analysis of SGA infants revealed the following associations: S-PE (odds ratio [OR] 3.226, 95% confidence interval [CI] 1.385–7.515; adjusted OR [AOR] 3.675, 95% CI 1.467–9.207; p    6.5%) (OR 0.436, 95% CI 0.187–1.017; AOR 0.459, 95% CI 0.179–1.173; p  > 0.05); low levels of high-density lipoprotein cholesterol (HDL-C  0.05). Multivariate analysis of LGA revealed the following associations: S-PE (OR 30.885, 95% CI 0.398–2.013; AOR 0.974, 95% CI 0.400–2.371; p  > 0.05); high levels of HbA1c (OR 4.542, 95% CI 0.187–11.824; AOR 3.997, 95% CI 1.452–10.998; p  < 0.05); low levels of HDL (OR 3.393, 95% CI 1.362–8.453; AOR 2.900, 95% CI 1.100–7.647; p  < 0.05). Conclusions The results of our analysis revealed that severity of preeclampsia was associated with SGA. The high HbA1c and low HDL-C values found in our analysis were independent risk factors for LGA in women with GCP, while other lipoproteins were not associated with abnormal NBW. These findings suggest that there are differences in the effects of various maternal lipid parameters on NBW.

              Changing trends of adverse pregnancy outcomes with maternal age in primipara with singleton birth: A join point analysis of a multicenter historical cohort study in China in 2011-2012.

              Adverse pregnancy outcomes are related to two opposite maternal factors, youth and aging. However, the change in trend of specific outcomes with childbearing age is unknown. The aim of this study was to investigate the changing trend of various maternal and neonatal outcomes with maternal age from 17 to 44 years.
