      Model to Predict Septicemia in Diarrheic Calves

          Abstract

          The difficulty in distinguishing between septicemic and nonsepticemic diarrheic calves prompted a study of variables to predict septicemia in diarrheic calves, 28 days old that were presented to a referral institution. The prevalence of septicemia in the study population was 31%. Variables whose values were significantly different (P < .10) between septicemic and nonsepticemic diarrheic calves were selected using stepwise, forward, and backward logistic regression. Variables identified as potentially useful predictors were used in the final model‐building process. Two final models were selected: 1 based on all possible types of predictors (laboratory model) and 1 based only on demographic data and physical examination results (clinical model). In the laboratory model, 5 variables retained significance: serum creatinine > 5.66 mg/dL (> 500 μmol/L) (odds ratio [OR] = 8.63, P = .021), toxic changes in neutrophils ≥ 21 (OR = 2.88, P = .026), failure of passive transfer (OR = 2.72, P = .023), presence of focal infection (OR = 2.68, P = .024), and poor suckle reflex (OR = 4.10, P = .019). Four variables retained significance in the clinical model: age ≤ 5 days (OR = 2.58, P = .006), presence of focal infection (OR = 2.45, P = .006), recumbency (OR = 2.98, P = .011), and absence of a suckling reflex (OR = 3.03, P = .031). The Hosmer—Lemeshow goodness‐of‐fit chi‐square statistics for the laboratory and clinical models had P‐values of .72 and .37, respectively, indicating that the models fit the observed data reasonably well. The laboratory model outperformed the clinical model by a small margin at a predictabilty cutoff of 0.5, however, the predictive abilities of the 2 models were quite similar. The low sensitivities (39% and 40%) of both models at a predicted probability cutoff of 0.5 meant many septicemic calves were not being detected by the models. The specificity of both models at a predicted probability cutoff of 0.5 was .90%, indicating that .90% of nonsepticemic calves would be predicted to be nonsepticemic by the 2 models. The positive and negative predictive values of the models were 66–82%, which indicated the proportion of cases for which a predictive result would be correct in a population with a prevalence of septicemia of 31%.

          Development of a scoring system for the early diagnosis of equine neonatal sepsis.

          A sepsis scoring system was developed and tested prospectively in a blind study of 190 neonatal foals admitted to the University of Florida Veterinary Medical Teaching Hospital's neonatal intensive care unit. The system used 14 readily available historical, clinical or laboratory variables and weighted each item to arrive at a sepsis score. The score was found to have a sensitivity of 93 per cent, a specificity of 86 per cent, positive accuracy rate of 89 per cent and negative accuracy rate of 92 per cent. The sepsis score was far more sensitive and specific for infection, even in very early cases, and had fewer false positive and false negative values than did any parameter taken individually.

            Dairy calf management, morbidity and mortality in Ontario Holstein herds. III. Association of management with morbidity

            Associations between heifer calf management and morbidity, particularly scours and pneumonia, were studied on 104 randomly selected Holstein dairy farms in southwestern Ontario between October 1980 and July 1983. At the farm level, data were stratified by season, with two six-month seasons (winter and summer) per year. The odds of farms with particular management strategies having above median morbidity were calculated. At the individual calf level, the odds of a calf being treated, controlling for farm of origin and month of birth, were calculated for different management practices. Farm size, and policies related to anti-scour vaccination, offering free-choice water and minerals to calves, methods of feeding, and the use of medicated feeds significantly altered the odds of a farm experiencing above-median pneumonia rates. Farm policies with regard to anti-scour vaccination, offering free-choice salt to calves, age at teat removal, type of calf housing, and use of preventive antimicrobials significantly altered the odds of a farm experiencing above-median scours rates. Scours and pneumonia were significantly associated with each other at both the farm and the calf level. No significant associations were found between individual calf management practices and the odds of being treated for scours. Sire used, method of first colostrum feeding, navel treatment, use of anti-scour vaccine in the dam, and the administration of preventive antimicrobials significantly altered the odds of a calf being treated for pneumonia.

              Use of a clinical sepsis score for predicting bacteremia in neonatal dairy calves on a calf rearing farm.

              In human, equine, and bovine neonates, early diagnosis of bacteremia remains a challenge for the internist. The objective of this study was to develop a predictive model for risk of bacteremia, based on a clinical evaluation system called the clinical sepsis score. Blood from 90 ill calves, 1- to 14-days-old from a calf-raising farm in the San Joaquin Valley of California was cultured. The calves were also scored according to a clinical score for hydration status, fecal appearance, general attitude, appearance of scleral vessels, and umbilical abnormality. Age, rectal temperature, heart rate, respiratory rate, and presence or absence of a focal site of infection were recorded. Prevalence of bacteremia was 31% (28/90). A logistic regression model indicated that high clinical score, presence of a focal infection, and increased age were associated with an increased risk of bacteremia in ill calves (P or = 40.8% were considered bacteremic, yielding acceptable sensitivity (75%) and specificity (71%) estimates. The predictive model was validated through a 2nd sampling of 100 calves (79 ill calves and 21 controls), of which 17 calves were bacteremic. The classification was 75% correct using the model, with an estimated sensitivity of 76% and specificity of 75%. Overall, results indicated that the model could be a useful tool for predicting bacteremia in ill calves in a clinical setting.
