      New Perspectives in Nonintrusive Sleep Monitoring for Neurodegenerative Diseases—A Narrative Review

          Abstract

          Good sleep quality is of primary importance in ensuring people’s health and well-being. In fact, sleep disorders have well-known adverse effects on quality of life, as they influence attention, memory, mood, and various physiological regulatory body functions. Sleep alterations are often strictly related to age and comorbidities. For example, in neurodegenerative diseases, symptoms may be aggravated by alterations in sleep cycles or, vice versa, may be the cause of sleep disruption. Polysomnography is the primary instrumental method to investigate sleep diseases; however, its use is limited to clinical practice. This review aims to provide a comprehensive overview of the available innovative technologies and methodologies proposed for less invasive sleep-disorder analysis, with a focus on neurodegenerative disorders. The paper intends to summarize the main studies, selected between 2010 and 2022, from different perspectives covering three relevant contexts, the use of wearable and non-wearable technologies, and application to specific neurodegenerative diseases. In addition, the review provides a qualitative summary for each selected article concerning the objectives, instrumentation, metrics, and impact of the results obtained, in order to facilitate the comparison among methodological approaches and overall findings.

          The Pittsburgh sleep quality index as a screening tool for sleep dysfunction in clinical and non-clinical samples: A systematic review and meta-analysis.

          This review appraises the process of development and the measurement properties of the Pittsburgh sleep quality index (PSQI), gauging its potential as a screening tool for sleep dysfunction in non-clinical and clinical samples; it also compares non-clinical and clinical populations in terms of PSQI scores. MEDLINE, Embase, PsycINFO, and HAPI databases were searched. Critical appraisal of studies of measurement properties was performed using COSMIN. Of 37 reviewed studies, 22 examined construct validity, 19 - known-group validity, 15 - internal consistency, and three - test-retest reliability. Study quality ranged from poor to excellent, with the majority designated fair. Internal consistency, based on Cronbach's alpha, was good. Discrepancies were observed in factor analytic studies. In non-clinical and clinical samples with known differences in sleep quality, the PSQI global scores and all subscale scores, with the exception of sleep disturbance, differed significantly. The best evidence synthesis for the PSQI showed strong reliability and validity, and moderate structural validity in a variety of samples, suggesting the tool fulfills its intended utility. A taxonometric analysis can contribute to better understanding of sleep dysfunction as either a dichotomous or continuous construct.

            The Emerging Evidence of the Parkinson Pandemic

            Neurological disorders are now the leading source of disability globally, and the fastest growing neurological disorder in the world is Parkinson disease. From 1990 to 2015, the number of people with Parkinson disease doubled to over 6 million. Driven principally by aging, this number is projected to double again to over 12 million by 2040. Additional factors, including increasing longevity, declining smoking rates, and increasing industrialization, could raise the burden to over 17 million. For most of human history, Parkinson has been a rare disorder. However, demography and the by-products of industrialization have now created a Parkinson pandemic that will require heightened activism, focused planning, and novel approaches.

              The metabolic consequences of sleep deprivation.

              The prevalence of diabetes and obesity is increasing at an alarming rate worldwide, and the causes of this pandemic are not fully understood. Chronic sleep curtailment is a behavior that has developed over the past 2-3 decades. Laboratory and epidemiological studies suggest that sleep loss may play a role in the increased prevalence of diabetes and/or obesity. Current data suggest the relationship between sleep restriction, weight gain and diabetes risk may involve at least three pathways: (1) alterations in glucose metabolism; (2) upregulation of appetite; and (3) decreased energy expenditure. The present article reviews the current evidence in support of these three mechanisms that might link short sleep and increased obesity and diabetes risk.
